Cooperation between hospital and homecare professionals via telecommunication equipment, to ensure safe and smooth release from hospital and ensure safe transition period.

Cooperation between different service providers: data sharing among health professional and telecare service professionals to provide a better knowledge about the situation of the users and improve their quality of life.
